The topic starter is referring specifically to the door and evacuation slide on a 747 classic crew service door (upper deck), NOT the 747-400. Perhaps you could find a video on youtube that depicts that arrangement. I can't find anything.

The door appears to be identical across the 747 line per the ACAPS, which makes sense as there would be no reason to change it.

Yes, if you're referring to freighters. The 744 upper deck passenger aircraft doors are much different. Maybe I'm belittling this topic a little too much...Next subject.
