Never left the airport
CLE (When it was still a hub during the CO/UA Merger days)
CLT
Chicago (Both airports)
CVG
DEN: This is an odd one, because while I left the airport to stay at an hotel after missing my flight to MCO (thanks a lot Frontier!), I never went into the city limits.
DFW
DTW
EWR: I've been to NYC many times (never flown into EWR for a NYC Trip), but I've never been to Newark.

Airports that I've transited without ever leaving the airport or visiting the city:
AMS, CDG, FCO, DFW, DTW, MIA, YYC, CLT

Honorable mentions:
-- DEN, LAS: I've visited both cities on road trips, and flown through both airports, but I've never actually flown to either city as a destination.
-- BTS: I flew in to BTS, and immediately boarded a bus to Vienna. The bus did stop in Bratislava, but I didn't really visit the city.
-- JFK and MDW: Transited both, but always arrived at other airports (LGA and ORD) when visiting those cities.

My list is pretty short since my most visited cities happen to be DL hubs!
MSP and SIN
I will fly to DFW this October, so I am not going to include it.

I have actually been to ATL/SLC/CVG/DTW, I noticed many here have not!
Next: CVG-CDG-ATH (Delta Air Lines B767-300ER, Air France A319)

Aha, I actually never left Narita. Travelled on a "buddy pass" type deal with Virgin back in the 90s. Took the bus into town from the crew hotel and got drunk (up the stairs from the train station and into the labyrinth of back streets), thinking I was in Tokyo.

For now, I have just one such airport on my list; ZRH. YUL was also there two decades ago, but I later visited Montreal in 2007. Same with VIE, though I visited the city twice before flying through there.

ATL. I guess I am not the only one.

Not a hub, but I have flown many times in and out of Charleroi (CRL) and never visited the city. I am curious though as sometimes it is said to be the ugliest city in Europe :)

Also while I have visited Munich a couple of times by train and car, every time I have used MUC has been only connecting, so I have never been on the "landside" of the airport or got in or out from the city.

HKG, BAH, CMB -> On transit. Did not leave the airport.
FRA/HHN -> Entered into FRA but had to catch a connecting flight from HHN. So hired a one way car rental, drove to HHN and took the connecting flight. Not sure if this constitutes of me visiting the city of Frankfurt.

Never left the plane - FRA, CUS and BSB.

Left the plane but not the airport BKK, LIN, ORD, CGB and BVA

Never left the plane: DKR

Never left the airport: ZRH, NRT, TLV

A couple of exceptions: visited FRA and MEM only as a teenager, visited MEX only once when I was 7, have only seen the parts of CLT close to the airport

Up until 2014 I had been through SIN multiple times without entering the city

Including now-defunct hubs, I've connected through CLT, PHL, PIT, and TPA (other than TPA, see something in common? ;)) but have never visited any of those cities proper.

SLC is the only one that comes to my mind for me. All other airports, I've visited the city at one time or another.

The interesting question though is what connection was routed via HAM?

Since I have that rule for myself that I go to the city and whenever I fly to the airport for the first time and only book flights that match that rule, there's really not a lot for me.

Places (not really hubs) I've connected at but didn't get out and have never been to on another occasion: ASP, BAH, POS, SDQ; soon: NBO. I usually connected via ADD or JNB and this time it's two extended connections at NBO but both during nighttime.
In Africa, there would also be a couple of airports that I didn't get off the plane since they were enroute stops.

I was flying from MUC to LHR once on Lufthansa and it worked out cheaper to route via HAM than fly direct. Also had my one and only go around so far in HAM as it was a particularly windy day.

Hahaha. I know you're being facetious, but I recently visited my niece/nephew in Atlanta. They're nearly finished building both a new NFL stadium and MLB stadium. There is a lively food scene and a well-received opera and symphony. No, it's not New York or any other major destination, but it's a worthy city in its own right.

Hah! I actually would like to visit sometime. It does actually seem like a pretty vibrant city. I've just never really had a reason to go out of my way to go there, especially as I've lived most of my life here on the west coast.

A very short list:

HKG - transited twice; once in 2007 on SQ's SIN-HKG-SFO route and once in 2017 when flying to YYZ on CX
ICN - SQ 015 stops here to refuel while returning from SFO
OSL & IST - both cities were short stops in a long itinerary ~ BOM-IST-OSL-BGO

I once was picked up in ATL and drove to Murphy, NC so never actually visited the Atlanta area, but used the airport as a destination point once. RDU was back in the AA Hub days.

Made it outside all the others.
if assumptions could fly, airliners.net would be the world's busiest airport
