Currently MS flies CAI-BOM and connections/pricing options to other Indian cities aren't optimal. Currently the other Gulf/Middle East airlines take the lion's share of the market.

The ride itself will take a few minutes. But you will need to be quick and not dilly dally.
T1 is quite a distance from T2 and T3 which are very close to each other.

EgyptAir domestic flights depart and arrive at Terminal 3, not 2.
Your best bet is the APM Automated People Mover.
Simply walk out of Terminal 1, find the APM, and get off at Terminal 3.
No airside shuttles unfortunately.

In the Middle East, the obvious example is DXB being at most 2 or 3 hours' drive from most of the country. My longest streak is 4 flights on 4 airlines, back in 2017:
29/10 EK DXB-CAI 77W
14/11 MS DXB-LHR 77W
17/11 UA LHR-IAD 787
22/11 OS IAD-VIE 767

5th flight, on 23/11 MS VIE-CAI, broke the streak

Usually most of my trips are round trips on the same airline, so I kind of got lucky here =)

Yes, they can. Provided they have a valid visa or ESTA, they are admissable to the US.
The best way is to fly nonstop from Africa or via a country that is not on the list of banned countries (basically Europe+ India+ China and a few others).
Via Dubai or Doha, or Istanbul should be fine as well.

Perhaps this route can be taken over by flydubai? They have a lot of secondary Italian destinations served with their 737. Unless BLQ does particularly well for some reason to justify year round WB service.

The 767 was used on the Gatwick to Dhahran (later Dammam) route in the 90s.
I also remember that sometime in the 2010s BA would schedule the 767 into DXB from LHR. Sometimes they would have 4 flights into Dubai during the winter schedule and the 1400 flight out of Dubai was a 767.
